I am completing this Guardian Consent & Attestation in order to enrol a minor (the "Child") at Good Climbs Inc. ("Good Climbs") and to allow the Child to participate in bouldering and the other activities, property, facilities, and services at Good Climbs (collectively, the "Activities"). This is a starting draft; edit it at `/admin/legal/guardian-waiver` to match your final policy. By signing on the Child's behalf I confirm each of the statements below.
I am the Child's parent or legal guardian
I confirm that I am the parent or court-appointed legal guardian of the Child, or another adult (18+) that the Child's parent or legal guardian has authorised in writing to enrol the Child and sign on their behalf. I have the legal authority to make this agreement for the Child, and I understand Good Climbs may ask me for identification or proof of guardianship.
The information I have provided is correct
I assure that the information I have provided about the Child — including their full name, date of birth, shoe size, any medical or physical conditions, and the emergency contact details — is true, accurate, and complete to the best of my knowledge. I will keep this information up to date and tell the front desk promptly if anything changes. I understand Good Climbs relies on this information for the Child's safety and that providing false or incomplete information may put the Child or others at risk.
I agree to the waiver and all policies on the Child's behalf
I have read, understood, and agree — for myself and on the Child's behalf — to the:

I accept, on the Child's behalf, the same assumption of risk, release, and responsibilities the Waiver describes, and I understand that a minor cannot sign their own liability waiver, so my signature is the binding record for the Child.
Assumption of risk & supervision
I understand that bouldering and the Activities carry inherent risks of injury, including serious injury, and that no protective measures can remove those risks. I voluntarily accept those risks on the Child's behalf. I agree to supervise the Child as required by the Parental / Kids Rules for their age, to remain reachable while the Child is in the gym, and to ensure the Child follows all staff instructions. I understand Good Climbs staff may pause the Child's climb or require closer supervision at their discretion for safety.
Medical & emergency authorisation
If the Child is injured or becomes ill and I cannot be reached immediately, I authorise Good Climbs staff to arrange first aid and any emergency medical care they reasonably believe is necessary, and I accept responsibility for the cost of that care. I confirm that I know of no medical condition that would prevent the Child from participating in the Activities, except any I have disclosed to Good Climbs.
Personal data of the Child
I consent, on the Child's behalf, to Good Climbs collecting and processing the Child's personal data for membership, safety, check-in, and the operation of the Activities, in line with the Privacy Policy and the Data Privacy Act of 2012 (RA 10173). I understand I may exercise the Child's data-privacy rights through Good Climbs.
Acknowledgement
I confirm that I have read this Guardian Consent & Attestation in full, that I understand it, and that I am signing it freely. This consent remains in effect for the Child's membership until the Child reaches adulthood, the guardianship is transferred or ended, or I withdraw it in writing, and it is binding on me, the Child, and our respective heirs and representatives.
